MADRID — Spain’s Ebola patient showed some marginal signs of recovery Sunday but European infectious diseases experts found shortcomings that need fixing in the Madrid hospital designated to deal with Ebola cases, officials said Sunday.
Assistant nurse Teresa Romero, who was infected with the Ebola in a Madrid hospital after having contact with two Spanish missionaries who later died from the virus, is showing signs of “slight improvement,” a government statement said.
But the statement also said that Romero’s prognosis remains serious and further complications can’t be ruled out.
The statement added that a committee from the European Center for Disease Prevention and Control who visited Madrid’s Carlos III hospital concluded that it wasn’t suitable to meet emergencies such as an Ebola outbreak...Fernando Simon, the Health Ministry’s coordinator of emergency alerts, said one sign for optimism was that “the virus load” in Romero’s bloodstream was decreasing.
Simon told a news conference at Prime Minister Mariano Rajoy’s official residence that he had met with the two-man European communicable disease threats committee and been told of its concerns.
He said the area that needed to be improved was the airlocks leading into and out of the quarantine area at the Carlos III hospital.
“The airlocks were set up to deal with highly infectious situations, but what hadn’t been foreseen was a need for bulky outfits to perform certain medical procedures in,” Simon said.
He said the airlocks had been deemed too tight for such protective apparel.
Health authorities have said they suspected Romero may have become infected when she accidentally touched her face with a gloved hand while taking off protective gear in a small airlock.

#5  Chairman again calls to bar entry for people from Ebola Hot Zone
WASHINGTON – House Homeland Security Chairman Michael McCaul called again Sunday for barring entry into the United States for people from countries wresting with the Ebola epidemic in West Africa.

“We need to target more the individual themselves and look at the idea of potentially temporarily suspending the 13,000 visas that would be coming out of this region,” the Austin Republican told CBS’ Face the Nation.

McCaul held a field hearing at Dallas/Fort Worth International Airport on Friday, after calling for a travel ban on Liberia, Sierra Leone and Guinea. Other Texas lawmakers and a number of conservative advocates have also demanded a travel ban. So have a handful of congressional Democrats.

At least bar entry for 'nonessential' travelers, like Thomas Eric Duncan had been. Health care workers, shippers of food & supplies would be excepted. Why those running for re-election to Congress are not being asked this question -- repeatedly -- is most interesting.

In the absence of effective vaccines and medication these protocols, of course are the only line of defense against an epidemic.

#8  Mount Sinai Health System in New York, which had two more suspected Ebola cases last week, has been going beyond the Centers for Disease Control and Prevention’s recommendations for taking off protective gear and using the World Health Organization protocol, which involves hand washing at more stages.

Most of the PPE shown in this video seems to utilize air-supplied head & shoulder hoods, but not the full-body, air-supplied, positive pressure protective suits. I really doubt either is being used in Dallas at the moment.

Texas Health Care Worker Tests Positive For Ebola
[NPR] A health care worker at the Texas hospital who was treating the Ebola patient at Texas Health Presbyterian Hospital, has tested positive for the virus, according to a statement from the Department of State Health Services.

The worker reported a low fever Friday night and was referred for testing at the state public health laboratory in Austin. The preliminary result was released late Saturday.

Centers for Disease Control in Atlanta will perform further tests to confirm the diagnosis.

"We knew a second case could be a reality, and we've been preparing for this possibility," said Dr. David Lakey, commissioner of the Texas Department of State Health Services, in the statement. "We are broadening our team in Dallas and working with extreme diligence to prevent further spread."

Officials are identifying the patient's contacts and will monitor them based on the nature of their interactions with the patient. Ebola is spread through direct contact with bodily fluids, and is not contagious before patients show symptoms such as fever.

The worker helped care for Eric Duncan, the 42-year-old who contracted Ebola in Nigeria and flew to Texas on Sept. 20. He first developed symptoms on Sept. 24.

Duncan died at Presbyterian Hospital on Wednesday.

#4  Transmission in spite of mask, face shield, gloves and gown. This is a frightening development. Either the PPE was defective or insufficient, or this may demonstrate that this strain of Ebola has become airborne.

If they were using the cheap paper gowns, that might be a problem. I've seem two types, the thicker shiney side ones, and the cheap almost see thru paper ones. Also, did they mention a glove year or similar failure? If so then that's the likely vector. If not, then we should be reexamining the Simmons on how this spreads.

#6  The PPEs, i.e. respirators and protective masks have to be inspected, tested and certified on a regular basis to insure proper function. I wonder if the hospital was keeping up with this aspect of their safety/hygiene program.

#9  Even if the protection gear is fully functional a simple mistake when removing contaminated protection is enough to cause an infection.

Doesn't the hospital keep videotape of their personnel performing highly sensitive tasks?
If so they could objectively review their procedures.

Another question:
Why aren't all persons under special surveillance generally tested for Ebola at fixed intervals?

This could both provide an early warning and help assess the sensitivity of these tests in human subjects.
Also do we actually know that there's no silent form of Ebola in humans where humans are infected and nearly asymptomatic but somewhat contagious for a time?

It looks like the 'top men' are passing on an opportunity to get actual hard data.

Their ER is probably being scrubbed down BIG TIME. Cannot do that and continue to see patients!
Other news articles this morning on this issue state there might be some staffing problems at Texas Health Presbyterian -- ? multiple no-shows by health care workers ? resignations.

ABC News chief health and medical editor Dr. Richard Besser recently returned from the Ebola hot zone in Liberia. He said Sunday morning that if the new Ebola case reported in Dallas is confirmed, it is a "very concerning" development....Besser said he does not agree with the Centers for Disease Control, which says any U.S. hospital can safely care for an Ebola patient.

"To do it safely, health care workers need to train and practice using protective equipment like they have been doing at the Emory and Nebraska facilities," he said, referring to special biocontainment units at Emory University Hospital in Atlanta — where Fort Worth physician Kent Brantly was treated for Ebola exposure; and the Nebraska Medical Center in Omaha, where an NBC photojournalist is currently being cared for. "I would never have gone into an Ebola ward in Africa without being dressed and decontaminated by experts — health care workers here should expect no less."

#17  Muddled US standards for handling Ebola spreading FUD:

Public health is the purview of the states, and as the nation anticipates more Ebola cases, some experts say the way the United States handles public health is not up to the challenge.

"One of the things we have to understand is the federal, state and local public health relationships," says Michael Osterholm, director of the Center for Infectious Disease Research and Policy at the University of Minnesota. "Public health is inherently a state issue. The state really is in charge of public health at the state and local level. It's a constitutional issue. "

Few seem to realize this.
Local, state & federal authorities assembled 30 Sept 2014 to create the Dallas Emergency Operations Center,
staffed by officials from Dallas County, the city of Dallas, the CDC, the county and state health departments and the Dallas County Sheriff's Department, among others.

This was the team that made decisions on matters such as isolating people who had been in direct contact with Duncan, including his fiancée, Louise Troh, her teenage son and two other male relatives. Because they were not sick, they couldn't technically be quarantined, Jenkins said Friday. Instead, Lakey issued a "control order" to keep them at home, where they could be monitored for signs of Ebola.


I have read countless examples from US history of contacts without symptoms being quarantined. Google "Typhoid Mary" Some of these cases required special court action &/or legislation, some did not. If Judge Jenkins is correct and there is no way to enforce a quarantine on asymptomatic Ebola contacts, the law needs to be changed NOW!

That is so true. Videotaping is a good idea to see where contamination is occurring. Some kind of decontamination/sterilization procedures may be required. The model used in the nuclear industry for PPEs, monitoring, decontamination, etc. is a good one. Get the damn pre-election politics out of this problem--they infest everything this administration does.

#20  Wearing those things for any amount of time or activity, especially in heat, is actually less fun than you think - knowing that it isn't much fun to begin with, especially as you increase your protection the more and more you end up inside a stiff ziplock bag.

Without training and a certain level of actual physical conditioning, people will get tired and make mistakes, especially under pressure.

DALLAS — Within hours after a test indicated that an employee at Texas Health Presbyterian Hospital had contracted the Ebola virus, Dallas officials knocked on doors, made automated phone calls and passed out fliers to notify people within a four-block radius of the health care worker's apartment complex about the situation, though they said there was no reason for neighbors to be concerned.

Dallas police officers stood guard outside the complex on Marquita Avenue Sunday and told people not to go inside. One said an industrial barrel outside contained hazardous waste taken from inside the building...By mid afternoon, a representative of CG Environmental spoke with reporters to say that a team of 15 hazardous materials workers would begin decontaminating the apartment of the hospital worker who fell ill.

The same company was contracted to disinfect the Dallas apartment where Thomas Eric Duncan, the nation's first Ebola patient, had stayed.

NJ enforces Ebola isolation order for NBC TV news crew after violation
AP reported Friday & Saturday that the State of New Jersey Department of Health has now issued a mandatory quarantine order for NBC News correspondent Dr. Nancy Snyderman and the other members of a NBC news crew exposed to a case of Ebola -- one of its cameramen, Ashoka Mukpo, while they worked together in Liberia.

Mukpo is now being treated at the Nebraska Medical Center in Omaha. The medical director there Saturday stated the patient's condition has improved.

The rest of the NBC crew returned to the USA by jet and, according to reports, had agreed to voluntary quarantine through 21 Oct. However, the planetprinceton.com website posted reports of Dr. Snyderman being seen in public at Hopewell Boro between 8 & 9 Oct.

"Unfortunately, the NBC crew violated this agreement and so the Department of Health today issued a mandatory quarantine order to ensure that the crew will remain confined until Oct. 22," the spokeswoman for the New Jersey Health Department told the Associated Press.
Left unstated was whether the sighting of Dr. Snyderman was the event that was considered a "violation" of the agreement.
NBC told the Associated Press today that the network could not comment on any individual case, but said the team was deemed to be low-risk upon its return from Liberia and its members agreed to follow guidelines set by local health authorities.

"We fully support those guidelines and continue to expect that they be followed," the representative said. "Our team are all well with normal temperatures, which they check multiple times a day, and they are also in daily contact with local health officials."

US, UK to test-model big bank collapse
(Rooters) - Regulators from the United States and the United Kingdom will get together in a war room next week to see if they can cope with any possible fall-out when the next big bank topples over, the two countries said on Friday.

Treasury Secretary Jack Lew and the UK's Chancellor of the Exchequer, George Osborne, on Monday will run a joint exercise simulating how they would prop up a large bank with operations in both countries that has landed in trouble. Also taking part are Federal Reserve Chair Janet Yellen and Bank of England Governor Mark Carney, and the heads of a large number of other regulators, in a meeting hosted by the U.S. Federal Deposit Insurance Corporation.

Six years after the financial crisis, politicians and regulators around the globe are keen to prove they have created rules that will allow them to let a large bank go under without spending billions in taxpayer dollars. They have forced banks to ramp up equity and debt capital buffers to protect taxpayers against losses, and have told them to write plans that lay out how they can go through ordinary bankruptcy. The plans are so-called living wills.

Because the failure of a big bank is such a rare event, regulators may not be used to talking to each other. There have also been suspicions that supervisors would first look to save the domestic operations of a bank, and would worry less about units abroad.

The exercise comes as regulators are about to bring to fruition further initiatives to make banking safer.

#5  This is more testing how Treasury and FDIC will handle the demise of a large bank and the ripples it sends thru the financial system. Especially with brokerages intertwined with banks these days, it could damage the stock markets, and credit markets, which would slam to a halt any economic growth.

One of the defendants, prior to their arrest in early September, told a television talk show that the video was recorded during a birthday party.

Relatives of the men, protesting their innocence or fearful of publicity, waited anxiously Saturday as the defendants met judges in chambers because of the sensitivity of the case, an AFP correspondent reported.

A lawyer and a judicial official told news hounds that the judges had decided to deliver the verdict on Nov. 1.

A brother of one defendant said: "We do not want the media to know anything about this case."

The mother of another said that "my son is innocent and the medical test has cleared him."

A front man for the justice ministry's forensics department also insisted Saturday that the men were innocent. "The medical test showed that the eight defendants have not practiced homosexuality recently or in the past," Hesham Abdel Hamed told AFP.

"The entire case is made up and lacks basis. The police did not arrest them red-handed and the video does not prove anything."

Secretive X-37B Military Space Plane Could Land in California Tuesday
The U.S. Air Force's mysterious X-37B space plane will return to Earth this week —possibly as early as Tuesday — after 22 months in orbit on a secret mission.
what, oh what, could it be?
The robotic X-37B space plane, also known as the Orbital Test Vehicle, will land at the Vandenberg Air Force Base in California, where Air Force officials are gearing up for its return. As of today (Oct. 12), the X-37B mini-shuttle has been in orbit since December 2012 and racked up a record-shattering 671 days in space.

"Team Vandenberg stands ready to implement safe landing operations for the X-37B Orbital Test Vehicle, the third time for this unique mission" said Col. Keith Baits, 30th Space Wing commander, in a statement on Friday

on Saturday said the second installment of Rs25,000 will be given to flood-affected families from October 20, said a report published on Radio Pakistain.

Addressing the flood-affected people at Jandyana area in Jhang district today, he said the first installment of Rs25,000 was paid to the flood-affected families before Eidul Azha.

The premier said the government will not rest till complete rehabilitation of flood-affected people, adding that all available resources have been accessible for relief activities.

"Damage assessment has been carried out and each and every flood-affected family will be fully compensated without any discrimination," he said.

Satellite images: Damage at the Military Research Complex at Parchin
[Israeli Defense] 24 hours after the reports from Iran regarding a mysterious explosion at the military compound in Parchin, evidence has been received that refutes the denials of the Iranian government.

Satellite images of the Parchin area, to the east of Tehran, prove: the explosion reported by the Iranian media had, indeed, occurred inside the military compound in Parchin, where, according to western intelligence agencies, trials are being conducted on nuclear missile fuzes. Satellite images obtained by Israel Defense and analyzed by specialist Ronen Solomon clearly show damage consistent with an attack against bunkers in a central locality within the military research complex at the Parchin military compound.

#1  Not sure about the "attack" assessment, but it certainly appears nuclear fusing charges may have ignited causing a chain reaction [possibly sympathetic detonation]. This tends to refute my ammo bunker theory while supporting Pappy's assessment from yesterday.
