#11 A fourth leak? I am wondering if this an unintentional blunder like Chernobyl...? Something like a pressurized pipe, under external water pressure, can function for long periods of time if left alone to operate at optimum pressure. Then somebody starts stresses it by 'shutting it down' and restarting -or- doing 'pressure checks' (like Chernobyl, perhaps).

As an initial operating hypothesis I favor 'happenstance' over diabolical plotting -- because mostly people are too stupid to do things Mother Nature can.
