#2  I read someplace that the Iranians kept adding 'stuff' to the vessel's superstructure and apparently didn't take into account the effect on the vessel's buoyancy.

That and 'leaving the door open' (kind of like dropping a cigarette in the ammo storage compartment) might signal a failure in human cognition.
