#2 Wasn't the reuse of the O rings; it was the use of them outside their intended environmental conditions; too cold and they got brittle.

#4 The rings did not weather well in Utah.
The loading propellant process for the solid fuel booster has a load procedure then a packing/compression cycle, with repeat. The compression forms variable density gradients in the fuel mix. More chloride makes for more O2 and a faster flash burn ergo more pressure. Firing of a fuel compression density ring coincident with a brittle cylinder gasket caused the blowout.
