#4 Maybe it is just me, but I'd less concerned with who f**ked up which part of this mess and more about what they could have done to get a better outcome and what they would do differently next time. You know, the whole After Action/Lessons Learned thing. At the end, you'd have a 3-ring binder with all those important little details like where to get master keys, who to call, and who is running the show - stuff you don't want to try and figure out on the fly.

Also, a bunch of people should be reconsidering their career choices and looking for something more suitable to their abilities and temperament.

Been there. However when dealing specifically with heath, safety, and life, there is no 'dealing' with those who f'up except by example.

#7 Accountability demands consequences when failure occurs. That does not happen in our gummint, constabulary or corporate worlds anymore. They make a show of still having it in the military, but it's not all on the up-and-up there.

#9 Regarding after action plan, the officer on scene ordered them to not follow the standard plan. They were in place super-fast and then stood there. The door was unlocked, they had ballistic shields, they waited until it was over. Heads should roll.
