#2 OK, so it's based on the HK416, which the Navy Seals use in preference to other variants of the same M4 derived carbine.

That means the lack of accuracy probably stems from one of the "improvements" made to the existing design. That happens a lot in the world of military firearms.

#4 I have no idea why any military would be firing commercial .223 rounds in their carbines. Those are chambered for 5.56x43 NATO.
