#3 We used to make fun of the Soviet addiction to demoting the junior officer that made the original mistake and their superior officers all the way to the regional overall commander. A very Asian practice that some ancient Chinese emperor would have approved of. The same Emperor would probably be heard complaining that ..."No one shows initiative! I have to order everything myself!"
