#2 COA 1. Install Craftsman C-clamps on all 14.5mm T&E mechanisms prior to future range firing events.

COA 2. Expend all ammunition prior to the gunner performance evalution phase.

COA 3. Institute individual crew self-evaluations.

That sounds very painful.

I blame Sly Stallone, in the "Rambo" trailer there's a scene where the hero messily blasts a truck driver at point blank range with an heavy (14,5mm?) MG - obviously, this poor man has been watching Evil Youtube, and his mind was poisoned by western entertainment. In fact, I blame bush, rather.

#6 The reservist , a veteran, was the No. 1 member of AA team( the one who pulls the trigger.) When his team got poor grade again, he blew the top, turned his gun around, and mowed down his evaluators.

The reservist training had all stopped due to the accident.

Accident. They called it an accident.
