#1 It's unclear what action, if any, warranted the flying of the Jolly Roger:

....One of those 'If we told you, we'd have to kill you' moments. And BTW, if you saw Tom Hanks' Greyhound, you saw the first Kidd (DD-661) - she's a museum now, and a good chunk of the movie was filmed aboard her.

#4 I've heard that on the last voyage of the USS George Washington SSBN598,she was restricted to shallow depths submerged because of her age. Subsequently, rammed and sank a Japanese freighter. She was denied the use of a Rising Sun flag on the periscope.
