#1 Navy scientist John P. Craven advanced much of the work on listening to subs at sea. His book is worth a read. The Silent War: The Cold War Battle Beneath the Sea. The Thresher and Scorpion incidents are covered, as well as the recovery of H-Bombs after the refueling accident over Spain.

#3 Given that there was no possible rescue system available at that point in time, and the depth in question, and that the fate of the crew was sealed, I can see it being a kindness to assert all dies instantly. Cold, dark asphyxiation seems a pretty grim exit to imagine for a loved one....better to thing of an instantaneous end.
