#1 Not saying this is the cause, but when stationed in a H-60 ASW squadron, we were witness to a Japanese Self-Defense Force H-60 shed a rotor blade upon touchdown at Barber’s Point: a/c immediately turned on its side and disintegrated. All survived. Shortly thereafter all -60s were downed for ndt inspections of rotor heads; records indicated there were 6 potentially bad units ( manufacturing flaws) out there.
