#2  In Tom Clancy's Red Storm Rising, one of those F-15 borne ASAT missiles was dusted off and used. They actually got a petite female pilot to fly the mission to get the last bit of altitude before launch.

Today they'd figure out how to launch an SM-6 from some sort of high-altitude / high-performance platform.
